What is the first question that most women have when they come to see you for breast augmentation?

Dr. Cohen (00:28):
I guess the first question that people have is my opinion about what size, well, actually, it's really whether saline or gel. I mean, the first question is what kind of implant you're going to get and are you good candidate? Some people do come in and think that they need an implant when they just need a lift and they have enough breast tissue, so I'll do it without an implant if I can. If there's no volume, you've had kids, you've lost weight, you have no volume, then an implant will really help give you that volume back. And there's saline and there's silicone. Saline's just filled with water and silicone, we use the gummy bear implants now, and those work very, very well, but they're filled with silicone. They have more cohesive gel silicone that if you cut it down the middle, it's like a gummy bear and that's why they call it a gummy bear implant.

Eva Sheie (01:11):
Do you have a method or a specific process for helping people decide whether they want saline or silicone?

Dr. Cohen (01:18):
I think my patients take my advice on my 23 years of experience as a board certified plastic surgeon. And if you have enough breast tissue, a lot of times these days I'm doing saline because if it were to break, you know it, and it's just water. So about 50% of my patients are doing saline and 50% silicone gel. If they don't have enough breast tissue and they're going to feel the bag a little bit more and want a softer breast, then I'll do silicone. So I show them pictures on the iPad and then we examine them.
